Medical Forms

All William Penn University Student-Athletes must have a valid athletic physical turned in to the Sports Medicine Department prior to participating in your sport. Physicals must be completed on the William Penn University Pre-Participation Physical Form which can be found here. Physicals must be completed by a Medical Doctor (M.D.), Doctor of Osteopathic Medicine (D.O.), Nurse Practitioner (N.P. or A.R.N. P.), or Physician’s Assistant (P.A.). Physicals that are completed and signed by a Chiropractor will not be accepted under any circumstance with no exceptions made. All of the below forms must be completed and reviewed by a member of the WPU Sports Medicine Staff before a student-athlete is cleared to participate in any WPU athletic activity.

Valid insurance is also required prior to participation on a William Penn University athletics team. All insurance forms and copies of insurance will be handled through the Business Office.
